This is done. To save time, I just renamed the "Best Date/Time" field to "Date of Birth" so I wouldn't have to restyle the form field.

Keep in mind, on the back end when viewing all entries, all form fills prior to today will show "Date of Birth", but at the time the user filled them in, the field was labeled "Best Date/Time".
