After thinking it over some more, I realize I've done this explanation thing for PatSat Surveys before. Duh. I still think the staff must be prepared to explain further (and friendlier) than what I've crafted in the attachment. 

How we've done it before, and now, cuz it makes total sense, is to include the intro as part of the form. I was able to move the Date line at the bottom up with the signature, tweeze out some line spacing throughout, and reconfigure the top of the page. This bought us the space to put a little explainer in at the top. See what you think.
