In addition to reskinning the site, we also need to make several changes because information will be out of date starting April 1. Here's one of the many to-dos!

We need to update the following forms on the forms page. The new forms are attached below in order
  1. New Patient Packet
  2. Notice of Privacy Practices
We need to REMOVE the form for: HIPAA Telehealth Disclosure
