Okay team, made calls to this group. As I told Kathy, only one per location & some issues with location numbers on website & the spreadsheet.

Northbrook & Skokie offices have same number.

Oak Park number on spreadsheet is disconnected. Number on website does
not connect. I called 4 times & it never goes through.

Westchester number is answered by Amity Health Medical Group & she informed me they there Thursday only ( called on Thursday) but they do not staff their phones.

Hinsdale_Ogden number from spreadsheet is fax number.
