Thank you for moving forward with our On-Site Marketing Plan and Audit Program. 

As we discussed, it is a “Patient History, Examination, Diagnosis, and Treatment Plan" for your business. Our team is excited to get started, but to do that, we’ll need your help.

  1. Please fill out this form to the best of your ability, and forward it to any other key decision makers who may want input.

  2. [OPTIONAL] The Staff SWOT Worksheet is for members of your staff to give their input into your strengths and weaknesses as a company. Feel free to forward these along to anyone who may be willing to participate.

  3. Please upload any and all assets related to these questions (past brochures, pamphlets, ads, photo assets and video files) to our dropbox folder: INSERT LINK HERE.  (Link will be provided by PM)
  4. Finally, our team can make one or more calls to discover how your phone staff handles new patient inquiries. We typically record the call(s) and provide written reports. However, we must get your permission to record the phone calls. To grant permission, please reply to this email with the following message (copy+paste):
    “I authorize Healthcare Success, LLC, to record mystery phone calls to our organization.”
    This warrants that you are authorized to give us permission to record phone calls on behalf your organization. Please provide phone number(s) and names of individuals who will be receiving the calls, as well as any important information regarding office scenarios, if available.


    We need these back no later than INSERT DATE HERE (one week).

Finally, I'd like to propose a few dates we have available for the onsite meeting and a "Report of Findings" meeting my team will give to you ahead of the visit. At this moment, I have these dates available:
